Your willingness to volunteer with us demonstrates a commendable commitment to making a positive impact on the lives of those with disabilities.

As a program provider, we must ensure that all our volunteers meets all the requirements, so to proceed with your application, we kindly request that you provide the following documents:

• A completed volunteer application form;

• A valid driver's license;

• Is at least 18 years of age;

• Background Checks ( Criminal History, EMR, NAR, LEIE, OIG)

• High school diploma or a certificate recognized by a state as the equivalent of a high school diploma; or

• Documentation of a proficiency evaluation of experience and competence to perform the job tasks and at least three written personal references.

• CPR & First aid training certificate (If applicable);

• Training and knowledge of the requirements related to Abuse, Neglect, and Exploitation at least annually there after.
